We receive your referral
Whether by online form, fax, or email — your referral lands with our intake team the moment it's submitted.

Our intake team contacts the client within one business day to introduce themselves, verify insurance, and start scheduling.

We schedule an intake
The client is paired with the right provider for their needs, location, and insurance — psychiatric evaluation, therapy, or both.

We loop you back in
With the client's permission, we coordinate care with you — sharing diagnosis, medication plan, and follow-up cadence as needed.

We deliver psychiatric evaluation, medication management, therapy, case management, MAT for substance use, and pharmacogenomic testing. Services are available in-person at our three Colorado clinics and by secure telehealth statewide.

How quickly will you reach out to my client?
Within one business day. Most clients are scheduled for an intake within a week — sometimes the same day if they call our walk-in clinic in Englewood on a Tuesday or Wednesday.
Do I need to send chart notes with the referral?
No — but they help. If you can attach recent chart notes, current medication list, or a brief clinical summary, we can match your client with the right provider faster.
Can I refer for case management only?
Case management is currently offered as part of comprehensive care to existing Elevate clients. We don't accept external case-management-only referrals at this time.
What ages do you accept?
We see children, adolescents, adults, and older adults — ages 5 and up. For psychiatric medication, therapy, and substance-use treatment.
Do you take Medicaid and Medicare?
Yes. We accept Health First Colorado (Medicaid), Medicare, Tricare, VA, and most major commercial plans. Self-pay and sliding-fee options are available for uninsured clients.
What if my client is in crisis?
For an active mental health crisis, please direct your client to call or text 988, or go to the nearest emergency department. We are an outpatient clinic and don't provide emergency or inpatient care.
