What this is

An honest look at the service.

A psychiatric evaluation is the foundation of any care plan at Elevate. It's a 60-minute, one-on-one conversation with one of our board-certified psychiatric mental health nurse practitioners.

We take time to understand your history, current symptoms, sleep, stress, relationships, past treatments, and what you actually want to be different. Nothing is rushed and nothing is assumed.

You leave with a working diagnosis (when it makes sense to give one), a concrete plan, and clear next steps — whether that's medication, therapy, case management, additional testing, or a combination.

What to expect

No guesswork — here's how it actually goes.

    Before your visit

    We send intake paperwork and verify insurance ahead of time. Plan a few minutes of forms before your appointment starts.

  2. 02

    The conversation

    A 60-minute one-on-one covering history, symptoms, goals, and any concerns. You're welcome to bring a family member if that helps.

  3. 03

    Your plan

    We end the visit with a written plan: recommended next steps, any prescriptions, referrals, and a follow-up timeline.

  4. 04

    Follow-up

    Most clients return for a follow-up within 2–4 weeks. We coordinate with your other providers (PCP, therapist) if you want us to.

How long does the first appointment last?

Plan on 60 minutes for the clinical conversation, plus a few minutes beforehand to finish intake paperwork.

Will I be prescribed medication on the first visit?

Sometimes, depending on the plan you and your provider build together. Some clients leave with a prescription; others choose to start with therapy, case management, or further evaluation before deciding.

Can I bring a family member?

Yes. Bringing a spouse, parent, or close friend is welcome and often helpful. For children, a parent or guardian is required.

Is the visit confidential?

Yes — your evaluation is protected under HIPAA. We share information with other providers only with your written permission.
