Payment, Insurance & Sliding Fee Discount Program

Elevate Mental Wellness is committed to making care accessible. We accept insurance when available and offer a Sliding Fee Discount Program for eligible patients based on household income and family size.

No patient will be denied care based on inability to pay. Sliding fee discounts are available to patients who qualify, including uninsured and underinsured patients, in accordance with applicable federal guidelines.

Patients may be asked to provide information about household income and family size to determine eligibility for discounted services. Our team can help you understand your options before your appointment.

No Surprises Act

Your right to a Good Faith Estimate.

Under federal law, you have the right to receive a Good Faith Estimate explaining the expected cost of any non-emergency care if you are uninsured or choosing not to use insurance.

We provide a written Good Faith Estimate at scheduling or upon request. If the actual bill is at least $400 more than the estimate, you may dispute the charge through the federal patient-provider dispute resolution process.
