An honest look at the service.

Mental health care doesn't happen in a vacuum. Insurance changes, housing instability, employment gaps, school accommodations — they all affect treatment. Case management is the connective tissue that keeps the rest from falling apart.

Our case managers work alongside your prescriber and therapist as part of your broader care team. They handle the calls, the paperwork, the follow-through — so you can focus on the work that matters in session.

Case management is offered to existing Elevate clients as part of comprehensive care. We don't currently accept external case-management-only referrals.

What to expect

No guesswork — here's how it actually goes.

  1. 01

    Introduction

    Your prescriber or therapist refers you to a case manager based on what you're working on. Initial meeting is 30–45 minutes.

  2. 02

    Coordination

    Your case manager loops in the right people — schools, employers, benefits offices, other providers — only with your permission.

  3. 03

    Follow-through

    Regular check-ins until the practical pieces are in place. They show up, they call back, they track loose ends.

  4. 04

    Handoff

    When the logistical work is done, case management tapers off. Your clinical care continues uninterrupted.

Questions we hear often.

Is case management billed separately?

Coverage varies by plan. For most Medicaid plans, case management is covered; for commercial plans, we check coverage in advance.

Can I request case management directly?

Case management is currently offered to existing Elevate clients only. If you're not yet a client, start with a psychiatric evaluation or therapy intake.

How is this different from therapy?

Therapy is clinical work on emotions, behavior, and relationships. Case management is practical, logistical, real-world support outside the therapy room.
