Your rights as a patient
Elevate Mental Wellness is committed to treating every patient with dignity, transparency, and respect. Here is what you can expect from us as your care partner.
As a patient of Elevate Mental Wellness, you have the right to:
- Receive information about your rights before or at the beginning of services, whenever possible.
- Be treated with courtesy, dignity, respect, and timely attention to your needs.
- Participate in decisions about your care and treatment.
- Receive services that are appropriate to your individual needs.
- Receive understandable information about your diagnosis, recommended treatment, available alternatives, expected benefits, risks, and costs.
- Ask questions and receive clear answers about your care.
- Provide informed consent before treatment or services are provided.
- Accept or refuse medication, testing, procedures, or treatment, except when otherwise permitted or required by law.
- Receive care in a safe and least restrictive setting appropriate to your needs.
- Be free from abuse, neglect, exploitation, discrimination, retaliation, and the improper use of restraint or seclusion.
- Know the names, roles, and professional credentials of the individuals providing your services.
- Be informed when students, interns, or trainees may participate in your care and decide whether to allow their involvement.
- Request a second opinion or consultation when appropriate.
- Receive continuity of care and reasonable notice and assistance when services must be transferred or discontinued.
- Have your care coordinated with other healthcare professionals when clinically appropriate and authorized.
- Expect privacy and confidentiality of your information, except when disclosure is permitted or required by law.
- Access or request copies or summaries of your records as permitted by law.
- Have applicable medical or psychiatric advance directives included in your record and honored when legally appropriate.
- Be informed of any financial interest or conflict of interest that may influence your care or referral.
- Receive an estimate of the cost of services upon request.
- Receive information in a language and manner you can understand.
- Request reasonable accommodations for a disability or communication need.
- Receive free language assistance and communication aids when required.
- Know whom to contact with questions, concerns, or complaints about your care.
- File a grievance or complaint without fear of retaliation, discrimination, or inappropriate interruption of services.
If you believe your rights have been violated
Please ask any Elevate staff member for our Grievance Information, follow Elevate’s Grievance Procedure, or review the posted Questions, Concerns, or Grievances notice. You may submit a grievance verbally or in writing. Assistance is available upon request.
Elevate Mental Wellness will not retaliate against anyone who raises a concern, requests assistance, or files a good-faith grievance or complaint.
